SEE SCHEDULE OINTEGRATED HEALTH PARTNERS OF SOUTHERN CALIFORNIA (FORMERLY PRIMARY CARE HEALTH NETWORK LLC) ("IHP") WAS INCORPORATED ON JUNE 8, 2015 AND BEGAN OPERATIONS IN JULY 2016. HCP IS THE SOLE MEMBER OF IHP.A POWERFUL ADVOCATE FOR PRIMARY HEALTH CARE SERVICE PROVIDERS AND THEIR PATIENTS, IHP SEEKS TO IMPROVE THE QUALITY OF HEALTH CARE OUTCOMES BY INTEGRATING THE EFFORTS OF OUR MEMBER HEALTH CENTERS AND DRIVING COSTS OUT OF THE HEALTH CARE SYSTEM.
