SFPHF ACTS AS A FISCAL SPONSOR AND PASSES THROUGH GRANTS TO VARIOUS PROJECTS THAT ARE HOUSED IN THE SAN FRANCISCO DEPARTMENT OF PUBLIC HEALTH, INCLUDING PROVIDING HOMELESS HEALTH CARE, COORDINATION CARE FOR FORMERLY INCARCERATED INDIVIDUALS, IMPROVING THE ENVIRONMENTAL HEALTH OF INDIVIDUALS AND THE COMMUNITY, PROMOTING HEALTHY LIFESTYLES, IMMUNIZATIONS FOR CHILDREN AND ADULTS, AND PREVENTING TRANSMISSIBLE DISEASES.
