PERSON CENTERED CARE COORDINATION:WE MET FOLKS WHERE THEY WERE AT IN LIFE TO PROVIDE THE SUPPORT THEY NEED TO ACHIEVE THEIR GOALS THROUGH INFORMATION & ASSISTANCE, OUR PERSONAL ASSISTANT REGISTRY, INDEPENDENT LIVING SKILLS & PEER SUPPORT AND BENEFITS NAVIGATION. ADDITIONALLY, DURING COVID-19 ESTABLISHED A TEAM OF CARE GIVERS TO WORK IN RESIDENTIAL CARE FACILITIES.


HOUSING PRESERVATION:PROVIDED A RANGE OF SUPPORT TO INDIVIDUALS WHO ARE PRECARIOUSLY HOUSED AND AT RISK OF LOSING THEIR HOUSING THROUGH HOUSING REFERRAL AND NAVIGATION, HOME MODIFICATIONS, ASSISTIVE TECHNOLOGY, LANDLORD & TENANT FACILITATION, AND NURSING HOME TRANSITION ASSISTANCE & SERVICE COORDINATION. ADDITIONALLY, DURING COVID-19 ASSEMBLED AND DELIVERED PPE TO LOW-INCOME HOUSING FACILITIES FOR DISTRIBUTION TO THE RESIDENTS. IN PREPARATION FOR THE PUBLIC SAFETY POWER SHUT OFFS, DEPLOYED HIGH VOLTAGE BATTERIES TO CLIENTS TO POWER LIFESAVING MEDICAL DEVICES.


ADVOCACY & ASSISTANCE:WE ASSISTED WITH INDIVIDUAL, AND SYSTEMS CHANGE ADVOCACY AND OUR CLIENT FINANCIAL ASSISTANCE PROGRAMS, THE BOB ROBERTS CLIENT ASSISTANCE FUND AND THE MARY MATHEWS BREAST CANCER ADVOCACY PROJECT. ADDITIONALLY, DURING COVID-19 PROVIDED ADDITIONAL FINANCIAL ASSISTANCE FOR RENT/GROCERIES AND OTHER ESSENTIAL NEEDS. PROVIDED CHROMEBOOKS TO DETER SOCIAL ISOLATION.
