AGING & DISABILITY RESOURCE CONNECTION: WE ASSISTED WITH INDIVIDUALS WITH ENHANCED INFORMATION & REFERRAL SERVICES, OPTIONS PLANNING, SHORT TERM SERVICE COORDINATION AND PERSON-CENTERED TRANSITION/DIVERSION SUPPORT.
PERSON CENTERED CARE COORDINATION:WE MET FOLKS WHERE THEY WERE AT IN LIFE TO PROVIDE THE SUPPORT THEY NEED TO ACHIEVE THEIR GOALS THROUGH INFORMATION & ASSISTANCE, OUR PERSONAL ASSISTANT REGISTRY, INDEPENDENT LIVING SKILLS & PEER SUPPORT AND BENEFITS NAVIGATION. ADDITIONALLY, DURING COVID-19 ESTABLISHED A TEAM OF CARE GIVERS TO WORK IN RESIDENTIAL CARE FACILITIES.
ADVOCACY & ASSISTANCE:WE ASSISTED WITH INDIVIDUAL, AND SYSTEMS CHANGE ADVOCACY AND OUR CLIENT FINANCIAL ASSISTANCE PROGRAMS, THE BOB ROBERTS CLIENT ASSISTANCE FUND AND THE MARY MATHEWS BREAST CANCER ADVOCACY PROJECT.
