ASSISTANCE IS PROVIDED TO PERSONS WHO ARE WILLING AND ABLE TO MOVE INTO A MORE INDEPENDENT LIVING SITUATION IN THE COMMUNITY. SPOKANE CENTER FOR INDEPENDENT LIVING PROVIDES A TRANSITIONAL PROGRAM THAT IS DESIGNED TO ASSIST INDIVIDUALS RESIDING IN INSTITUTIONAL LIVING SITUATIONS TRANSFER TO A MORE INDEPENDENT, LESS RESTRICTIVE COMMUNITY-BASED SETTING. A QUALIFIED INDEPENDENT LIVING SERVICES CASE WORKER WILL WORK ONE-ON-ONE WITH YOU, EXPLORING OPTIONS TO HELP YOU FIND THE RIGHT INDEPENDENT LIVING SITUATION FOR A SUCCESSFUL TRANSITION. IN ADDITION, THE CASE WORKER WILL ASSESS YOU HEALTHCARE REQUIREMENTS TO ENSURE CONTINUITY OF CARE IS MAINTAINED.
