SULIK ASSISTANCE FUND FOR EMPLOYEES, INC. (SAFE) PROVIDES NEEDS-BASED ASSISTANCE TO EMPLOYEES OR FAMILIES OF EMPLOYEES OF THE COUNTRY CLUB OF FAIRFIELD (CCF). EMPLOYEES MAY SEEK GRANT ASSISTANCE FROM SAFE THROUGH A WRITTEN REQUEST TO THE BOARD OF DIRECTORS OF SAFE BY COMPLETING AN APPLICATION FORM. DETERMINATION OF QUALIFYING NEED FOR ASSISTANCE IS MADE BY THE BOARD OF DIRECTORS OF SAFE, WHICH INCLUDES URGENT MEDICAL CARE OR MEDICAL EXPENSE ASSISTANCE, EXPENSES RELATED TO MEDICAL INSURANCE DEDUCTIBLES AND/OR OUT OF POCKET EXPENSES, HARDSHIP DUE TO EXTENDED LOSS OF INCOME, EXPENSES RELATED TO FAMILY BEREAVEMENT, LOSS OR DAMAGE TO PERSONAL PROPERTY OR RESIDENCE, TRAVEL COSTS OR TIME AWAY TO CARE FOR INFIRM OR GRAVELY ILL IMMEDIATE FAMILY MEMBERS, AND TUITION ASSISTANCE FOR JOB-RELATED EDUCATION OR TRAINING. THE BOARD OF DIRECTORS OF SAFE IS INDEPENDENT OF CCF; IT IS COMPRISED OF THREE INDIVIDUALS WHO ARE PAST PRESIDENTS OF CCF BUT ARE NEITHER CURRENT EMPLOYEES, OFFICERS OR DIRECTORS OF CCF. A 100% OF THEIR TOTAL TIME IS DEDICATED TO THIS MISSION. THE PRIMARY SOURCE OF FUNDS TO SUPPORT S.A.F.E. WILL BE FROM THE MEMBERS OF THE COUNTRY CLUB.
