FORM 990, PART III, LINE 4A, PROGRAM SERVICE ACCOMPLISHMENTS:NOVAS OUTPATIENT PROGRAM PROVIDED SPECIALIZED SERVICES TO CLIENTS TOMEET THEIR INDIVIDUAL NEEDS WHICH PROMOTED HEALING AND LASTING RECOVERY.SERVICES INCLUDED INDIVIDUAL AND GROUP COUNSELING AS WELL AS ANINTENSIVE SUBSTANCE ABUSE RECOVERY PROGRAM. INDIVIDUAL AND GROUPCOUNSELING PROVIDED CLIENTS WITH SUPPORT AND TOOLS THAT THEY NEEDED TOADDRESS ANXIETY, DEPRESSION, TRAUMA AND OTHER ISSUES IN ORDER TOIMPROVE THE QUALITY OF THEIR LIFE AND DAY-TO-DAY FUNCTIONING. NOVAOFFERED A 36-HOUR INTENSIVE SUBSTANCE ABUSE OUTPATIENT PROGRAM AS WELLAS A 72-HOUR INTENSIVE PROGRAM. BOTH PROGRAMS ASSISTED CLIENTS INDEVELOPING THE NECESSARY SKILLS TO ABSTAIN FROM ALCOHOL AND OTHERDRUGS. THE PROGRAM ALSO PROVIDED EDUCATION AND SUPPORT SERVICES TOFAMILY MEMBERS AND SIGNIFICANT OTHERS IN ORDER TO STABILIZE THE FAMILY UNIT AND EFFECTIVELY INTEGRATE THE INDIVIDUAL INTO A SUPPORTIVE,COMMUNITY-BASED RECOVERY ENVIRONMENT. PROFESSIONALS PROVIDED ASSISTANCE,SUPPORT AND EDUCATION AT THREE VALLEY CLINICS TO BEHAVIORAL HEALTHRECIPIENTS WITH DUAL DISORDERS. SERVICES INCLUDED PROVIDING EDUCATIONON SUBSTANCE ABUSE RECOVERY, MENTAL HEALTH SYMPTOMS AND WAYS TO HELPRECIPIENTS DEVELOP THE SKILLS NECESSARY TO IMPROVE DAILY LIFE. CRISISTRANSITION NAVIGATORS ASSISTED INDIVIDUALS AFTER A CRISIS SITUATION.FOLLOWING A CRISIS, INDIVIDUALS WERE REFERRED TO PROVIDERS COUNTRYWIDE,INCLUDING NOVA, TO RECEIVE SERVICES FROM A PEER WHO WAS KNOWLEDGEABLEABOUT COMMUNITY RESOURCES AND HAD FIRSTHAND EXPERIENCE REGARDINGTRIUMPHING OVER SIMILAR CHALLENGES. THEY WERE THEN GUIDED TO THEPROGRAMS AND SERVICES THEY NEEDED. BOARD CERTIFIED PSYCHIATRIC MENTALHEALTH NURSE PRACTITIONERS AT NOVA PROVIDED PSYCHIATRIC EVALUATIONS ANDONGOING MED MANAGEMENT SERVICES. NOVA, INC. CEASED ACTIVE OPERATIONS AND TRANSFERED CARE OF IT'S CLIENTS TO TERROS, INC. ON APRIL 1, 2014. THE ORGANIZATION IS IN THE PROCESS OF WINDING-DOWN.

TREATMENT SERVICES WERE OFFERED TO CHEMICALLYDEPENDENT MEN AND WOMEN, PREGNANT WOMEN, AS WELL AS DUALLY DIAGNOSED INDIVIDUALS. NOVA, INC. CEASED ACTIVE OPERATIONS AND TRANSFERED CARE OF IT'S CLIENTS TO TERROS, INC. ON APRIL 1, 2014. THE ORGANIZATION IS IN THE PROCESS OF WINDING-DOWN.

NOVAS SAFE HAVEN SOUGHT TO PROVIDE ASSISTANCE TOTHESE INDIVIDUALS WHO HAD BEEN UNWILLING OR UNABLE TO PARTICIPATE INOTHER SUPPORTIVE SERVICES, ASSISTING THEM TOWARD STABILITY ANDINDEPENDENCE. THE NOVA SAFE HAVEN RESIDENTIAL PROGRAM HOUSED 25 PEOPLE(18 MEN AND SEVEN WOMEN). ANOTHER 25 INDIVIDUALS WERE ENGAGED IN THEDAY SERVICES PROGRAM. BOTH THE RESIDENTIAL AND DAY PROGRAMS ENGAGEDINDIVIDUALS AND PREPARED THEM TO ACHIEVE THE GOALS OF INCREASEDSUFFICIENCY, STABILITY AND GREATER SELF-DETERMINATION-ALL OF WHICH WOULDLEAD TO PERMANENT HOUSING. IN ADDITION TO SHELTER, NOVA SAFE HAVENPROVIDED ITS CLIENTS WITH CASE MANAGEMENT, NUTRITION EDUCATION,COMMUNITY MEETINGS, THE AVAILABILITY OF AA AND NA AS WELL AS INDEPENDENT LIVING SKILLS.
