CARE COORDINATION SERVICES: EBENEZER HEALTHCARE ACCESS THROUGH ITS CARE COORDINATION SERVICES, SERVES AS A BRIDGE TO HEALTHCARE THAT CONNECTS UNDERSERVED COMMUNITIES, SPECIFICALLY IMMIGRANTS IN DAYTON, COLUMBUS AND CINCINNATI, OHIO. EHA'S COMMUNITY HEALTHCARE WORKERS EVALUATE THE HEALTH, NEEDS AND LIVING CONDITIONS OF CLIENTS AND THEIR FAMILIES. IN 2023 EBENEZER HEALTHCARE ACCESS SERVED 1,185 CLIENTS, INCLUDING 442 ADULTS, 716 PEDIATRIC CLIENTS AND 27 PREGNANT CLIENTS. FOR THOSE CLIENTS, 187 REFERRALS WERE MADE TO PRIMARY CARE PROVIDERS AND 124 CLIENTS WERE CONNECTED TO HEALTH INSURANCE, IN ADDITION, 5,350 SOCIAL SERVICE REFERRALS WERE MADE FOR SUCH SERVICES AS FOOD ASSISTANCE, TRANSPORTATION, BASIC NEEDS SUPPLIES, WOMAN, INFANT AND CHILDREN (WIC), AMONG OTHER REFERRALS FOR BASIC NEEDS.
