WE HAVE EIGHT CLINICS LOCATED IN GREELEY AND EVANS (WELD COUNTY) AND TWO CLINICS LOCATED IN LOVELAND (LARIMER COUNTY). SUNRISE OFFERS COMPREHENSIVE, QUALITY, AND AFFORDABLE MEDICAL, DENTAL, AND BEHAVIORAL HEALTH SERVICES INCLUDING ON-SITE LABORATORY, RADIOLOGY, PHARMACY, AND PATIENT EDUCATION SERVICES. COLLABORATIONS FOCUS ON INTEGRATED SERVICE EXPANSION; SHARED HIT INFRASTRUCTURE; WORKFORCE DEVELOPMENT; COMMUNITY HEALTH IMPROVEMENT; REGIONAL ACCOUNTABLE CARE ACTIVITIES; AND SYSTEM ACCOUNTABILITY AND EFFICIENCY. DURING THE FISCAL YEAR, SUNRISE SUPPORTED THE HEALTH OF 40,503 PATIENTS THROUGH 160,033 VISITS, INCLUDING 21,329 VIRTUAL VISITS. 89% LIVED AT OR BELOW 200% OF THE FEDERAL POVERTY LEVEL (FPL); 57% LIVED AT OR BELOW 100% OF FPL; 26% WERE UNINSURED AND 50% HAD MEDICAID. 43% SPOKE A DIFFERENT LANGUAGE THAN ENGLISH AS THEIR PRIMARY LANGUAGE.
