BEACON COMMUNITY CONNECTIONS, INC. PROVIDES NON-CLINICAL CASE MANAGEMENT, COMMUNITY CARE NAVIGATION UPON REFERRAL FROM COMMUNITY PARTNERS. IN 2021, BEACON RECEIVED 1367 REFERRALS AND WORKED WITH 1325 CLIENTS TO CONNECT THEM WITH COMMUNITY RESOURCES TO RESOLVE SOCIAL DETERMINANT OF HEALTH NEEDS. 90% OF CLIENTS WERE CONNECTED TO COMMUNITY RESOURCES SUCH AS TRANSPORTATION, HOUSING, MEDICINE, PUBLIC ASSISTANCE BENEFIT ENROLLMENT, AND MORE. ONLY 6% OF CLIENTS ARE READMITTED TO THE HOSPITAL WITHIN 30 DAYS WHILE ENROLLED IN BEACONS SERVICES.
BEACON COMMUNITY CONNECTIONS, INC. COORDINATED THE JUSTICE AND HEALTH COLLABORATIVE, A MULTI-AGENCY, CROSS SYSTEM INITIATIVE TO DEVELOP POLICY AND PROGRAMS TO PREVENT INDIVIDUALS WITH BEHAVIORAL HEALTH PROBLEMS FROM BECOMING ENTANGLED WITHIN THE CRIMINAL JUSTICE SYSTEM. BEACON ALSO COORDINATED THE LAFAYETTE REENTRY COALITION WHICH FOCUSES ON BUILDING A COMPREHENSIVE COMMUNITY RESPONSE TO FORMERLY INCARCERATED INDIVIDUALS.
BEACON COMMUNITY CONNECTIONS, INC. HOSTS THE ANNUAL SOUTH LOUISIANA COMMUNITY HEALTH SUMMIT. THIS EDUCATIONAL EVENT PROVIDES CONTINUING EDUCATION CREDIT OPPORTUNITIES TO NURSES, SOCIAL WORKERS, AND COUNSELORS AND OTHER HEALTH PROFESSIONALS AND COMMUNITY STAKEHOLDERS. IN 2021, OVER 150 PEOPLE ATTENDED THE EDUCATIONAL PORTION OF THE SUMMIT. 500 PEOPLE ATTENDED THE COMMUNITY HEALTH FESTIVAL WHICH WAS HELD IMMEDIATELY AFTER THE SUMMIT. THIS EVENT WAS FOR THE GENERAL PUBLIC AND WAS AN OPPORTUNITY TO EDUCATE THE PUBLIC ON SOCIAL DETERMINANTS OF HEALTH AND RELATED RESOURCES.
