TEMPORARY EMERGENCY HOUSING FOR VICTIMS OF ELDER ABUSE OR AT-RISK ADULTS.(VIBRANCE SENIOR) 4 CLIENTS SERVED. 1 REFERRAL FROM DEPT OF CORRECTIONS, 2 REFERRALS FROM EL PASO COUNTY ADULT PROTECTIVE SERVICES. 1 REFERRAL LAKE COUNTY ADULT PROTECTIVE SERVICES. THESE INDIVIDUALS WERE RESCUED FROM IMMEDIATE HOMELESSNESS, WHILE SAFE, LONG TERM SOLUTIONS WERE SOUGHT. OUTCOMES: 1 CLIENT TO INDEPENDENT APARTMENT, 1 CLIENT REUNITED WITH AND HOUSED BY FAMILY IN CHICAGO AREA, 1 CLIENT TO ASSISTED LIVING, 1 CLIENT TRANSITIONED TO VIBRANT SENIOR CARES PERMANENT HOUSING PROGRAMS.
STABLE AFFORDABLE LONGER-TERM HOUSING FOR INDIVIDUALS IN HOUSING (OUR VIBRANT ACHIEVER) ALTHOUGH THE HOUSING IS LONGER TERM, THE GOAL IS FOR THE ACHIEVER TO MOVE FORWARD. THIS PROGRAM IS CURRENTLY RENTING TO A CLIENT ORIGINALLY REFERRED FOR TEMPORARY EMERGENCY FROM DEPT OF CORRECTIONS. DIAGNOSED WITH HUNTINGTONS WITH CHOREA, THIS INDIVIDUAL IS BEST SERVED AT VIBRANCE UNTIL THE DISEASE PROGRESSES. A SECOND CLIENT MOVED FROM VIBRANCE TO A HOST HOME ACCEPTING MEDICAID.
