HOME HEALTH CARE: AZRA ELDERLY HOME HEALTHCARE IS A NONPROFIT HOME HEALTH AGENCY UNDER CPLC HEALTH, INC. THE GOAL OF AZRA IS TO PROVIDE THE NECESSARY AT HOME MEDICAL SUPPORT TO THE AGING POPULATION IN ARIZONA. SOME OF THE HOME HEALTH SERVICES WILL INCLUDE HEALTH EDUCATION; CHRONIC DISEASE MANAGEMENT; MEDICATION RECONCILIATION; WOUND CARE; AND PHYSICAL, OCCUPATIONAL OR SPEECH THERAPY. THESE SERVICES WILL BE PROVIDED WITHIN CHICANOS POR LA CAUSA'S (CPLC), RELATED ENTITY, HUD 202 SENIOR HOUSING PROPERTIES AND LOW INCOME COMMUNITIES IN PHOENIX. AZRA HAS A STATE LICENSE TO OPERATE AS A HOME HEALTH AGENCY AND IS CURRENTLY IN THE PROCESS OF OBTAINING MEDICARE AND MEDICAID CERTIFICATION; CHAP ACCREDITATION; AND CONTRACTS WITH COMMERCIAL HEALTH INSURANCE PLANS TO BILL FOR HOME HEALTH SERVICES.
