COMMUNITY HEALTH SERVICES - KESWICK MULTI-CARE HAS CREATED KESWICK COMMUNITY HEALTH, INC. A COMMUNITY HEALTH RESOURCE ORGANIZATION (CHRO) THE GOAL OF WHICH IS TO DEVELOP AN INTEGRATED HEALTH RESOURCE FOR OLDER ADULTS AND THEIR CAREGIVERS THAT INCREASES THEIR ABILITY TO LIVE AT HOME AND MEET THEIR HEALTH GOALS. THE KESWICK CHRO STRATEGIES INCLUDE DELIVERING A CHRONIC DISEASE PREVENTION AND MANAGEMENT PROGRAM WITH A FOCUS ON DIABETES, FALLS PREVENTION, AND COGNITIVE FUNCTIONING; PROVIDING EFFECTIVE CARE TRANSITION COORDINATION TO HELP ENABLE CLIENTS TO LIVE AT HOME AND STAY HEALTHY FOR AS LONG AS POSSIBLE; AND, ADDRESSING THE SOCIAL DETERMINANTS OF HEALTH THROUGH EFFECTIVE PARTNERSHIPS AND DIVERSE PROGRAM EXPERIENCES BOTH ON THE KESWICK CAMPUS AND IN THE COMMUNITY.
