THE ORGANIZATION PROMOTES PHILANTHROPY & CHARITABLE GIVING TO PROVIDE RESOURCES AND SUPPORT TO CHRISTUS MOTHER FRANCES HOSPITAL - SULPHUR SPRINGS AND SURROUNDING COMMUNITIES.

HOPKINS COUNTY HEALTH CARE FOUNDATION (THE "FOUNDATION") IS A TEXAS NON-PROFIT ORGANIZATION SERVING THE PHILANTHROPIC NEEDS OF CHRISTUS HOPKINS HEALTH ALLIANCE (THE "HOSPITAL"). IT IS THE FOUNDATION'S RESPONSIBILITY TO ENCOURAGE AND RECEIVE CONTRIBUTIONS FROM INDIVIDUALS,CORPORATIONS, ASSOCIATIONS, AND FOUNDATIONS. THESE CONTRIBUTIONS ARE DISTRIBUTED TO THE HOSPITAL AND ITS AFFILIATED FACILITIES,IN ACCORDANCE WITH THE INTENTIONS OF THE DONORS AND WITHIN THE CONFINES OF THE FOUNDATION'S GOALS AND OBJECTIVES. ALL CONTRIBUTIONS ASSIST THE HOSPITAL IN CONTINUING TO PROVIDE HIGH QUALITY HEALTH CARE IN AREAS SUCH AS MODERNIZATION AND CONSTRUCTION OF THE HOSPITAL FACILITIES AS NEEDED, ACQUISITION OF HOSPITAL EQUIPMENT, AND OTHER SPECIAL HEALTH CARE NEEDS OF THE HOSPITAL. THE FOUNDATION PROVIDED FUNDS TO THE HOSPITAL DURING FISCAL YEAR 2020 FOR CAPITAL EQUIPMENT. THE FOUNDATION'S SOLE CORPORATE MEMBER IS CHRISTUS HOPKINS HEALTH ALLIANCE; WHICH IS OWNED 51% BY CHRISTUS HEALTH AND 49% BY HOPKINS COUNTY HOSPITAL DISTRICT. CHRISTUS HEALTH WAS FORMED IN 1999 TO STRENGTHEN THE 153-YEAR-OLD, FAITH-BASED HEALTH CARE MINISTRIES OF THE CONGREGATIONS OF THE SISTERS OF CHARITY OF THE INCARNATE WORD OF HOUSTON AND OF SAN ANTONIO. FOUNDED ON THE MISSION "TO EXTEND THE HEALING MINISTRY OF JESUS CHRIST," CHRISTUS IS CHALLENGED TO REACH OUT TO, AND BEYOND, THE MORE THAN 160 COMMUNITIES WE SERVE TO HELP THOSE IN NEED. THE VISION OF CHRISTUS HOPKINS HEALTH ALLIANCE, AS A CATHOLIC, FAITH-BASED MINISTRY ORGANIZATION, IS TO BE A LEADER AND ADVOCATE IN CREATING EXEMPLARY HEALTH CARE SERVICES, PROCESSES AND STRUCTURES THAT IMPROVE THE HEALTH OF INDIVIDUALS AND OF LOCAL AND GLOBAL COMMUNITIES SO THAT ALL MAY EXPERIENCE GOD'S HEALING PRESENCE AND LOVE. CHRISTUS HOPKINS HEALTH ALLIANCE RESPONDS TO HEALTH CARE NEEDS THROUGH SERVICES PROVIDED AT CHRISTUS MOTHER FRANCES HOSPITAL - SULPHUR SPRINGS, A 96-BED ACUTE CARE HOSPITAL, AND AT THE CHRISTUS TRINITY CLINIC CAMPUSES IN SULPHUR SPRINGS.
