Part 3 - Line 1
DRC SOLUTIONS' (FORMERLY KNOWN AS THE DAY RESOURCE CENTER FOR THE HOMELESS) MISSION IS TO PROVIDE RESPECTFUL, RESPONSIBLE, AND EFFECTIVE COMMUNITY-BASED SOLUTIONS TO HELP INDIVIDUALS AND FAMILIES EMERGE FROM HOMELESSNESS AS PRODUCTIVE, HEALTHY PEOPLE.
Describe the Organization's Program Activity:
Part 3 - Line 4a
HOUSING CASE MANAGEMENT-THE DRC PROVIDES SPECIFIC PROGRAMS THAT FOCUS EXPLICITLY ONIDENTIFYING CHRONICALLY HOMELESS HOUSEHOLDS AND THEN PREPARING THESE INDIVIDUALS ANDFAMILIES EXPERIENCING HOMELESSNESS FOR RE-HOUSING, PLACEMENT INTO APPROPRIATE HOUSING (SUBSIDIZED AND UNSUBSIDIZED) AND ON-GOING SUPPORTS TO ENSURE LONG-TERM HOUSING PLACEMENT. THESE PROGRAMS ARE REFERRED TO AS HOUSING FIRST PROGRAMS AND ARE PERMANENT SUPPORTIVE HOUSING (PSH). DRC CASE MANAGERS WORK WITH FORMERLY CHRONICALLY HOMELESS HOUSEHOLDS TOWARDS HOUSING STABILIZATION AND SELF-SUFFICIENCY. THE DRC HELPS PARTICIPANTS IN ATTAINING NEEDED IDENTIFYING CRITICAL DOCUMENTS (STATE ID, SOCIAL SECURITY CARD, BIRTH CERTIFICATE), AND HELP RESIDENTS MEET THEIR DESIRED GOALS, SUCH AS MAINTAINING THEIR HOUSING, OBTAINING, OR INCREASING INCOME, AND ADDRESSING HEALTH, AND BEHAVIORAL HEALTH NEEDS. DRC CASE MANAGEMENT OFTEN SERVES AS THESE HOUSEHOLDS PRIMARY SUPPORT, AND ADVOCATE AS THEY REINTEGRATE INTO HOUSING AND THE BROADER COMMUNITY.
MOBILE CASE MANAGEMENT/OUTREACH AND NAVIGATION-THE DRC OPERATES THE MAJORITY OF HOUSING FOCUSED STREET OUTREACH AND NAVIGATION EFFORTS IN TARRANT COUNTY, RECEIVING REFERRALS FROM COMMUNITY PARTNERS, BUSINESS OWNERS, AND LOCAL OFFICIALS. DRC ENGAGES INDIVIDUALS AND FAMILIES EXPERIENCING UNSHELTERED HOMELESSNESS THROUGHOUT TARRANT COUNTY AND SCREENS ALL INDIVIDUALS FOR HOUSING, POSSIBLE DIVERSION, OR FAMILY REUNIFICATION. IF DIVERSION OR FAMILY REUNIFICATION IS POSSIBLE, DRC STAFF QUICKLY ASSESS AND REMOVE THE HOUSEHOLD FROM UNSHELTERED HOMELESSNESS. THE DRC THEN BEGINS THE PROCESS OF PREPARING THE PARTICIPANT FOR HOUSING BY ASSESSING, GATHERING CRITICAL DOCUMENTS (IDENTIFICATION, SOCIAL SECURITY CARD, BIRTH CERTIFICATE), AND VERIFYING HOMELESS EPISODES THROUGH ONGOING OUTREACH CONTACTS, AND/OR SUPPORTING DOCUMENTATION. MONTHLY OUTREACH CONTACTS ARE COMPLETED BY DRC STAFF FOR ALL PARTICIPANTS ENCOUNTERED, ENSURING THAT ACTIVE HOMELESSNESS IS DOCUMENTED AND THAT PROGRAM ELIGIBILITY REQUIREMENTS WILL NOT BE A BARRIER. DRC THEN RECEIVES HOUSING REFERRALS FOR THESE INDIVIDUALS AND FAMILIES THROUGH OUR LOCAL CONTINUUM OF CARE. DRC CONNECTS WITH HOUSING PROGRAMS, PROVIDING TRANSPORTATION TO INTAKE APPOINTMENTS, AND ENSURING THE INDIVIDUAL IS SUCCESSFULLY HOUSED. THE GOAL OF THE PROGRAM IS TO ASSIST INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS IN MULTIPLE WAYS. FIRST, BY MAKING SURE THEY ARE ENROLLED IN THE CONTINUUM OF CARE AND REGULARLY CHECK IN SO THAT THEY STAY ACTIVE AND ARE ELIGIBLE TO RECEIVE HOUSING ASSISTANCE. SECOND, BY HELPING THEM TO OBTAIN ANY NECESSARY CRITICAL DOCUMENTS NEEDED FOR THE HOUSING PROCESS. AND THIRD, BY ASSISTING IN LOCATING AN APPROPRIATE UNIT ONCE THEY RECEIVE HOUSING ASSISTANCE.
