ROCKY MOUNTAIN OPTIONS FOR LONG TERM CARE PROVIDES SINGLE ENTRY POINT (SEP) SERVICES FOR EL PASO, TELLER, AND PARK COUNTIES. THE SEP PROGRAM ASSISTS ELIGIBLE MEDICAID CLIENTS BY PROVIDING ACCESS TO HOME AND COMMUNITY BASED SERVICES IN ORDER TO REMAIN INDEPENDENT AND IN THEIR OWN HOMES. DURING 2018 3,457 CLIENTS WERE SERVED.
