NACES CONTRACTED WITH THE TX DEPARTMENT OF AGING AND DISABILITIES SERVICES (DADS) IN FISCAL YEAR 2016-2017 TO CONDUCT SURVEYS AS PART OF A FEDERALLY FUNDED DADS PROJECT: QUALITY OF LIFE (QOL). THE PROJECT IS A VOLUNTARY PROGRAM THAT ALLOWS RESIDENTS OF NURSING FACILITIES, STATE SUPPORTED LIVING CENTERS AND INTERMEDIATE CARE FACILITIES TO MOVE TO A MORE INDEPENDENT LIVING ENVIRONMENT AND RECEIVE SERVICES FROM DADS TO ALLOW THEM TO STAY IN THE LESS INSTITUTIONAL SETTING. THE PROJECT REQUIRES A FACE TO FACE QUESTIONNAIRE TO BE CONDUCTED PRIOR TO THE MOVE TO THE NEW SETTING AND FOLLOW UP FACE TO FACE SURVEYS AT 11 AND 24 MONTHS FROM THE INITIAL QUESTIONNAIRE. THESE SURVEYS ARE COMPLETED BY A REGISTERED NURSE, LICENSED VOCATIONAL NURSE OR SOCIAL WORKER. THERE WERE 302 SURVEYS COMPLETED IN FISCAL YEAR 2016-2017. THIS PROJECT IS NOW COMPLETE.
