INDEPENDENT LIVING CASE MANAGEMENT (ILCM) PROVIDES PERSONALIZED SERVICES AND INDIVIDUALIZED SUPPORT TO CONSUMERS TO RESOLVE PROBLEMS AND CONFRONT ISSUES AFFECTING THEIR INDEPENDENCE. THROUGH A CONSUMER-DIRECTED PROCESS THEY ACQUIRE ADVOCACY SKILLS TO TAKE CHARGE OF THEIR LIVES AND LIVE WELL IN THEIR COMMUNITY. ANOTHER COMPONENT OF THIS PROGRAM IS CONNECTING PEOPLE TO COMMUNITY RESOURCES THROUGH INFORMATION AND REFERRAL ASSISTANCE. FUNDING SOURCES: TITLE VII PART B FEDERAL GRANT, STATE OF CO GENERAL FUND, CITY OF FORT COLLINS, CITY OF LOVELAND, STATE OF COLORADO MEDICAID HEALTH CARE POLICY AND FINANCE, PRIVATE FOUNDATIONS/GRANTS, FUNDRAISING REVENUE, CONTRIBUTIONS (PRIVATE AND CORPORATE).
MEDICAL EQUIPMENT LOAN CLOSET (MELC) MAKES DURABLE MEDICAL EQUIPMENT AVAILABLE FOR SHORT-TERM USE AT A NOMINAL RENTAL FEE OR FOR FREE IF THE CONSUMER IS INDIGENT.
