THE PLAN PROVIDES HEALTH, DENTAL, VISION, AND LIFE INSURANCE BENEFITS COVERING SUBSTANTIALLY ALL EMPLOYEES OF CENTRAL NATIONAL BANK AND TRUSTCOMPANY, WHO CHANGED THEIR LEGAL NAME TO STRIDE BANK, NATIONAL ASSOCIATION (THE BANK, OR STRIDE BANK) EFFECTIVE FEBRUARY 16, 2019 AND CENTRAL SERVICE CORPORATION (THE COMPANY). THE PLAN IS SUBJECT TO THE PROVISIONS OF THE EMPLOYEE RETIREMENT INCOME SECURITY ACT OF 1974 (ERISA).

IN ADDITION TO HEALTH, DENTAL, VISION AND PHARMACEUTICAL BENEFITS, FULL-TIME EMPLOYEES THAT WORK 40 OR MORE HOURS PER WEEK ARE ELIGIBLE FOR LIFE INSURANCE BENEFITS. DEPENDENTS OF EMPLOYEES ARE COVERED FOR HEALTH, DENTAL, VISION AND PHARMACEUTICAL CLAIMS THROUGH PREMIUMS WITHHELD FROM ALL PARTICIPANTS' WAGES, INCLUDING OFFICERS.
