NURSE FAMILY PARTNERSHIP:CONTRACTORS PROVIDE PROGRAM RN'S AND SUPPORT PERSONNEL FOR DEPARTMENT OF HEALTH AND HOSPITALS' OFFICE OF PUBLIC HEALTH BUREAU OF FAMILY HEALTH THROUGH A PREVENTION, COUNSELING AND SERVICE LEARNING PROGRAM BEING IMPLEMENTED IN COMMUNITY BASED SETTINGS FOR LOW INCOME PARENTS WITH NEWBORN INFANTS.
PRIMARY CARE & RH: THIS PROGRAM PROVIDES PROGRAM SUPPORT PERSONNEL TO HEART DISEASE AND STROKE PREVENTION, STATEWIDE DIABETES PREVENTION AND CONTROL PROGRAM AND PROVIDES HEALTH SYSTEMS DEVELOPMENT TECHNICAL SUPPORT.
