ORGANIZATION PROVIDES PRENATAL MEDICAL CARE FOR UNINSURED WOMEN, IDENTIFIES MOTHERS AND BABIES AT RISK, PROVIDES CARE COORDINATION AS NURSE-SUPERVISED HOME VISITING SERVICES TO 7,379 WOMEN AND BABIES TO PROMOTE HEALTHY BIRTHS, REDUCE INFANT DEATHS AND PREMATURE BIRTHS.ASSISTS WITH EARLY INTERVENTION, SUPPORT, EDUCATION AND REFERRAL/LINKAGE TO COMMUNITY HEALTH SERVICES. ENSURES ACCESS TO PRENATAL CARE BY PROVIDING GUIDANCE TO PREGNANT WOMEN IN SELECTING A HEALTHCARE PROVIDER, ASSISTING WITH SCHEDULING MEDICAL APPOINTMENTS, INSURING THE BABY HAS HEALTH INSURANCE AND MEDICAL HOME AND INCREASING PARTICIPATION IN WIC, FAMILY PLANNING SERVICES AND OTHER STATE AND FEDERAL PROGRAMS THEY MAY QUALIFY FOR.

THE NURSE FAMILY PARTNERSHIP (NFP) IS AN EVIDENCE BASED PROGRAM THAT PROVIDES HOME VISITS FROM REGISTERED NURSES TO LOW INCOME, FIRST TIME MOTHERS. PROGRAM OBJECTIVE IS TO BETTER EQUIP PARENTS WITH THE KNOWLEDGE, SKILLS AND TOOLS TO ASSIST THEIR CHILDREN IN BEING HEALTHY, SAFE AND READY TO SUCCEED IN SCHOOL.
