HOPWA SERVICES WHICH INCLUDE ALL GRANTS ON THE SEFA EXCEPT THOSE TITLED CONTINUUM OF CARE HOMELESS ASSISTANCE. THREE HOPWA GRANTS PROVIDE 5 HOUSING ASSISTANCE OPTIONS, ALL WITH SUPPORTIVE SERVICES TO IMPOVERISHED AND/OR HOMELESS INDIVIDUALS AND FAMILIES LIVING WITH HIV. THE HOPWA PROGRAMS ASSISTED 573 PERSONS IN FY 2020.
CONTINUUM OF CARE HOMELESS ASSISTANCE GRANTS OUTCOMES: COC PROVIDES PERMANENT HOUSING AND SUPPORTIVE SERVICES FOR HOMELESS AND CHRONICALLY HOMELESS INDIVIDUALS AND FAMILIES LIVING WITH DISABLING CONDITIONS. GRACE HOUSE PROVIDES THIS HOUSING TO WOMEN IN RECOVERY FOR ADDICTION AND MEN, WOMEN, AND FAMILIES LIVING WITH HIV. DURING 2020, GRACE HOUSE SERVED 101 INDIVIDUALS AND ADDITIONAL 22 FAMILY MEMBERS.
RYAN WHITE GRANT PROVIDES HOUSING SERVICES TO PEOPLE WITH HIV, TO IMPROVE HEALTH OUTCOMES, AND TO REDUCE HIV TRANSMISSION AMONG HARD-TO-REACH POPULATIONS. GRACE HOUSE ASSISTED 103 PERSONS WITH RYAN WHITE HOUSING ASSISTANCE DURING FY 2020.
UMMC/ASSC- THIS IS A PARTNERSHIP BETWEEN GRACE HOUSE AND UMC ALLOWING US TO PROVIDE RENT, UTILITY, AND OTHER ASSISTANCE AS NEEDED TO IMPROVE HEALTH OUTCOMES FOR PERSONS LIVING WITH HIV. GRACE HOUSE ASSISTED 116 HOUSEHOLDS THROUGH THIS PARTNERSHIP IN FY 2020.
