THIS IS ACCOMPLISHED THROUGH OWNERSHIP AND OPERATION OF SUBSIDIZED SENIOR HOUSING AND HANDICAPPED HOUSING AND BY PROVIDING ACCOUNTING, ADMINISTRATIVE AND OTHER SUPPORT SERVICES TO OTHER NON-PROFIT ENTITIES THAT PROVIDE SERVICES TO ELDERS IN NEED.

THE ORGANIZATION PROVIDES FOOD PREPARATION SERVICES UNDER SERVICE CONTRACTS TO URBAN JACKSONVILLE, INC.FRO THE MEALS ON WHEELS PROGRAM, THE CITY OF JACKSONVILLE FOR SENIOR CENTER CONGREGATE MEAL SITES AND TO CATHEDRAL GERONTOLOGY CENTER, INC FOR THEIR NURSING HOME RESIDENTS. DURING THE YEAR ENDED 9/30/21, THE FOOD SERVICES DEPARTMENT PREPARED AND PROVIDED 273,432 MEALS TO URBAN JACKSONVILLE, INC., 97,623 MEALS TO CATHEDRAL GERONTOLOGY CENTER, INC. AND 166,798 MEALS WERE ALSO PREPARED AND PROVIDED TO THE CITY OF JACKSONVILLE SENIOR CENTERS AND OTHER SENIOR MEAL SITES IN THE JACKSONVILLE AREA.
THE ORGANIZATION PROVIDES ADMINISTRATIVE AND BOOKKEPING SERVICES TO URBAN JACKSONVILLE, INC AND ITS VARIOUS OPERATING DIVISIONS, AS WELL AS TO THE THIRD PARTY OPERATION CATHEDRAL GERONTOLOGY CENTER, INC., WHICH LEASES A SKILLED NURSING FACILITY FROM THE ORGANIZATION.URBAN JACKSONVILLE, INC., ITS SUBSIDIARY HOME HEALTH AGENCY AND SUBSIDIARY CLAY COUNTY SENIOR SERVICES OF AGING TRUE ALL PAY MANAGEMENT AND BOOKKEEPING FEES TO THE ORGANIZATION OF A 70%-30% RATIO PROPORTIONED TO THE REVENUES EARNED, AS DOESTHE FOOD SERVICES DIVISION OF CFI. THE CATHEDRAL GERONTOLOGY CENTER, INC. PAYS A CONTRACT FEE OF 6.5% OF ITS ANNUAL REVENUE NET OF QUALITY ASSESSMENT FEES AND EXTRAORDINARY INCOME ITEMS SUCH AS STIMULUS PAYMENTS.
THE ORGANIZATION IS THE FISCAL AGENT FOR THE FLORIDA SELF DIRECTED CARE PROGRAM FOR DISTRICT 4. THEPROGRAM PROMOTES SELF-DETERMINATION, RECOVERY AND FULL INCLUSION OF PEOPLE WHO HAVE BEEN DIAGNOSEDWITH A MENTAL ILLNESS AND DEPEND ON GOVERNMENT SUBSIDIZED MENTAL WELLNESS SERVICES. FUNDS ARE UTILIZED TO PURCHASE PSYCHIATRIC AND MENTAL WELLNESS CARE, AS WELL AS ALTERNATIVE AND NON-TRADITIONAL SERVICES THAT RESULT IN THE SAME SUCCESSES AS TRADITIONAL MENTAL HEALTH SERVICES. DURING THE TWELVE MONTHS ENDED9/30/21, THE PROGRAM PURCHASED SERVICES FOR 87 PARTICIPANTS.
AMONG OTHER SERVICES, THE ORGANIZATION OWNS AND LEASES A 120 BED SKILLED NURSING FACILITY TO A THIRD-PARTY OPERATOR, CATHEDRAL GERONTOLOGY CENTER, INC. AND A MEDICAL OFFICE BUILDING TO A PHYSICIANS' GROUP OPERATING UNDER THE AUSPICES OF A LOCAL CORPORATE MEDICAL CENTER.
