SUNNYSIDE HEALTH SERVICES, INC. OPERATES A 60-BED LICENSED SKILLED NURSING FACILITY AND SERVED APPROXIMATELY 247 PATIENTS AND RESIDENTS DURING THE REPORTING PERIOD. SUNNYSIDE HAS ACHIEVED CMS'S TOP 5-STAR RATING FOR QUALITY OF CARE. UNDER THE DIRECTION OF A FULL-TIME DIRECTOR OF NURSING, STAFF PROVIDE RESIDENTS WITH PERSONALIZED CARE DESIGNED TO MEET THEIR INDIVIDUAL NEEDS. SUNNYSIDE PROVIDES SERVICES TO RESIDENTS THROUGH MEDICAID, MEDICARE AND PRIVATE PAY OPTIONS. THERE WERE 12,820 RESIDENT DAYS IN THE REPORTING PERIOD.
