TRIAD HEALTH PROJECT PROVIDES EMOTIONAL AND PRACTICAL SUPPORT TO INDIVIDUALS LIVING WITH HIV/AIDS, TO THEIR LOVED ONES, AND TO THOSE AT RISK FOR HIV/AIDS; IMPLEMENTS STRATEGIES TO EDUCATE THOSE AT RISK AND THE COMMUNITY ABOUT HIV/AIDS; AND ADVOCATES LOCALLY, REGIONALLY, AND NATIONALLY FOR INDIVIDUALS AND GROUPS INFECTED WITH OR AFFECTED BY HIV/AIDS.

CARE AND WELLNESS - SERVICES PROVIDE PRACTICAL AND EMOTIONAL SUPPORT TO HELP CLIENTS MANAGE THEIR HIV DISEASE, INCLUDING CASE MANAGEMENT, SUPPORT GROUPS, FOOD PANTRY AND MEALS, TRANSPORTATION AND ALTERNATIVE THERAPIES. 355 CLIENTS SERVED WITH 9,683 UNITS OF CASE MANAGEMENT, 11,314 CLIENT CONTACTS, AND $298,000 OF DIRECT FINANCIAL ASSISTANCE. OF THE 355 CLIENTS SERVED, 55 RECEIVED ONLY THE DAY CENTER'S PROGRAMS. 1,569 BAGS OF GROCERIES WERE DISTRIBUTED AND 2,512 BUS PASSES. THE DAY CENTER'S PROGRAMS PROVIDED 251 GROUP SUPPORT SESSIONS TO 5,023 DUPLICATED INDIVIDUALS. THERE WERE 3,001 DUPLICATED INDIVIDUALS WHO PARTICIPATED IN 80 ADDITIONAL THERAPEUTIC AND RECREATIONAL ACTIVITIES. THERE WERE 4,077 MEALS SERVED TO CLIENTS AT THE DAY CENTER.


DAY PROGRAMS - PROGRAMS HELD BY THE ORGANIZATION TO PROVIDE EDUCATIONAL AND EMOTIONAL SUPPORT TO THEIR CLIENTS AND FAMILIES.


PREVENTION AND WELLNESS - PREVENTION PROGRAMS EDUCATE INDIVIDUALS AND THE COMMUNITY AND FACILITATE HIV TESTING AT NON-TRADITIONAL TESTING LOCATIONS. 1,562 UNDUPLICATED INDIVIDUALS RECEIVED PREVENTION SERVICES. 1,277 RECEIVED FREE, CONFIDENTIAL HIV TESTING, AND APPROXIMATELY 148 INDIVIDUALS ATTENDED 36 HIV SEMINARS AND HEALTH FAIRS. APPROXIMATELY 29,909 CONDOMS WERE DISTRIBUTED TO INDIVIDUALS IN HIGH-RISK GROUPS ALONG WITH HIV/STD PREVENTION INFORMATION. INTERVENTIONS USING ONLINE SOCIAL NETWORKS INCREASED HIV TESTING AMONG MEN AT RISK FOR HIV.
