CHILES AVENUE IS LICENSED AS AN ICF/IID GROUP HOME SERVING ADULTS WITH INTELLECTUAL DISABILITIES WHO REQUIRE SUPPORT IN THE HOME AND IN THE COMMUNITY. THE SUPPORT TEAM INCLUDES A NURSE, PSYCHOLOGICAL ASSOCIATE, DIETICIAN, PHYSICIAN AND A PSYCHIATRIST. THE PROGRAM IS FUNDED BY MEDICAID AND IS SURVEYED ANNUALLY.
ST. DUNSTANS MANOR IS LICENSED AS A SUPERVISED LIVING HOME UNDER THE RULES GOVERNING 5600 GROUP HOMES. THE RESIDENTS IN THE HOME REQUIRE MODERATE LEVELS OF SUPPORT FROM STAFF TO COMPLETE VARIOUS ACTIVITIES OF DAILY LIVING. THE HOME IS FUNDED THROUGH MEDICAID AND STATE FUNDING. THE HOME SERVES 6 INDIVIDUALS. ADDITIONALLY, THE HOME HAS A GARAGE APARTMENT AND SERVICES ARE PROVIDED TO THE INDIVIDUALS RESIDING IN THE GARAGE APARTMENTS.
