Describe the Organization's Program Activity:

Part 3 - Line 4a

PARTNER FOR MENTAL HEALTH PROVIDES SYSTEM NAVIGATOR SERVICES TO RESIDENTS OF CHARLOTTESVILLE, ALBEMARLE, AND THE SURROUNDING CENTRAL VIRGINIA AREA. THESE SERVICES ARE INTENTIONALLY DIVERSE IN ORDER TO MEET THE UNIQUE NEEDS OF EACH CLIENT, BUT THEY ALL SHARE THE COMMON GOAL OF CREATING STABILITY FOR PERSONS WITH MENTAL ILLNESS TO HELP THEM LIVE HAPPIER, HEALTHIER, AND MORE FULFILLED LIVES. SYSTEMS NAVIGATORS FACILITATE ACCESS TO LOCAL MENTAL HEALTH SERVICES BY PROVIDING INFORMATION AND REFERRAL ABOUT AVAILABLE TREATMENT PROVIDERS FOR INDIVIDUALS WITH SIMPLE NEEDS. FOR CLIENTS WITH MORE COMPLEX SITUATIONS, WE PROVIDE INTENSIVE, FLEXIBLE, AND COMPREHENSIVE SUPPORT TO CONNECT THEM WITH PARTNER AGENCIES THAT PROVIDE NEEDED SOCIAL SERVICES (HOUSING, EMPLOYMENT, FOOD, MEDICAL CARE, ETC.) IN ADDITION TO MENTAL HEALTH TREATMENT. OF THE 200 CLIENTS WE WORKED WITH LAST YEAR, 90% ARE TRAUMA SURVIVORS, 80% LACKED INSURANCE, AND 70% WERE INVOLVED IN THE CRIMINAL JUSTICE SYSTEM. DESPITE THESE CHALLENGES, WHILE WORKING WITH US 85% OF OUR CLIENTS REPORTED MAKING SIGNIFICANT PROGRESS TOWARD THEIR RECOVERY GOALS. THIS IMPRESSIVE OUTCOME IS ATTRIBUTABLE TO THE HARD WORK OUR CLIENTS ARE WILLING TO DO TO IMPROVE THEIR LIVES AND TO THE INTENSITY, FLEXIBILITY, AND COMPREHENSIVENESS OF THE SUPPORT WE ARE ABLE TO PROVIDE.
