THE MISSION OF THE APHA FOUNDATION IS TO IMPROVE HEALTH BY INSPIRING PHILANTHROPY, RESEARCH AND INNOVATION THAT ADVANCES PHARMACISTS' PATIENT CARE SERVICES. THE APHA FOUNDATION LOOKS TO CREATE A NEW MEDICATION USE SYSTEM WHERE PATIENTS, PHARMACISTS, PHYSICIANS AND OTHER HEALTH CARE PROFESSIONALS COLLABORATE TO DRAMATICALLY IMPROVE THE COST EFFECTIVENESS AND QUALITY OF PATIENT HEALTH OUTCOMES.

SCIENCE AND RESEARCH - OUR RESEARCH PROJECTS ESTABLISH NEW MODELS OF PRACTICE FOR PHARMACISTS AND REDEFINE PATIENTS HEALTHCARE EXPERIENCE. WE CONNECT EACH PATIENT TO A PHARMACIST WHO EMPOWERS THEM TO BECOME SUCCESSFUL IN MANAGING HIS OR HER OWN HEALTH. OUR WORK VALIDATED THAT THE PHARMACISTS ARE ESSENTIAL TO HELPING PATIENTS ACHIEVE BETTER HEALTH OUTCOMES. THE FOUNDATION RESEARCH PROJECTS INCLUDE IMMUNIZATION REGISTRIES, DIABETES PATIENT CREDENTIALING, DIABETES PREVENTION, RAPID CYCLE INNOVATION, TRIGLYCERIDES AWARENESS AND IMPACT INSTITUTE.
AWARDS AND GRANTS - TO ENCOURAGE THE ENHANCEMENT OF PHARMACISTS SKILLS AND TO PROVIDE BETTER SERVICE TO THE PUBLIC IN THE AREA OF PHARMACEUTICAL CARE. THE FOUNDATION HAS THREE AWARD PROGRAMS: 1. INCENTIVE GRANTS - PROVIDES SEED FUNDING TO PHARMACISTS WHO WANT TO ESTABLISH OR SUPPORT ALREADY EXISTING PATIENT CARE SERVICES WITHIN THEIR PHARMACY PRACTICE.2. PINNACLE AWARDS - ANNUAL RECOGNITION PROGRAM FOR CONTRIBUTIONS TO HEALTHCARE QUALITY THROUGH THE MEDICATION USE PROCESS.3.
