OUR MISSION IS TO IMPROVE THE HEALTH AND WELLNESS OF PEOPLE WHO LIVE OR WORK IN HOWARD COUNTY, MD.
Describe the Organization's Program Activity:
GRANT MAKING-SINCE 1998, HORIZON FOUNDATION HAS INVESTED MORE THAN $74 MILLION IN COMMUNITY PROGRAMS, INCLUDING GRANTS TO OVER 394 ORGANIZATIONS. THE HORIZON FOUNDATION IS COMMMITTED TO IMPROVING HEALTH THROUGH INNOVATIVE INITIATIVES, COLLABORATIVE PARTNERSHIPS, STRATEGIC GRANTMAKING AND THOUGHTFUL ADVOCACY. WE FUND COMMUNITY NONPROFITS WHO CAN IMPROVE HEALTH OUTCOMES AND WORK TOWARDS HEALTH EQUITY. WE HAVE EXPANDED ACCESS TO MENTAL HEALTH SERVICES, INCREASED WALKING AND BIKING ACCESSIBILITY, MADE HEALTHIER FOOD/DRINKS MORE ACCESSIBLE AND AFFORDABLE, AND ASSISTED RESIDENTS IN NAMING A HEALTH CARE AGENT TO MAKE END OF LIFE CARE DECISIONS. IN ADDITION, WE HAVE STRENGTHENED THE CAPACITY OF COMMUNITY ORGANIZATIONS TO DISMANTLE LONGSTANDING BARRIERS TO GOOD HEALTH FOR COMMUNITIES OF COLOR AND IMPROVED THEIR ABILITY TO ADVANCE MOVEMENTS FOR SOCIAL CHANGE.
COMMUNITY BUILDING-HORIZON FOUNDATION COLLABORATES WITH DIVERSE PARTNERS TO IDENTIFY PRESSING, UNMET HEALTH AND WELLNESS NEEDS IN THE COMMUNITY AND TO CREATE CATALYTIC CHANGE THAT IS TANGIBLE, MEASURABLE AND SUSTAINABLE. HORIZON FOUNDATION CARES PASSIONATELY ABOUT THE COMMUNITY AND STRIVES TO BRING INNOVATIVE SOLUTIONS FROM AROUND THE REGION, COUNTRY AND WORLD TO IMPROVE HEALTH AND WELLNESS LOCALLY.
