GRANT MAKING-SINCE 1998, HORIZON FOUNDATION HAS INVESTED MORE THAN $68 MILLION IN COMMUNITY PROGRAMS, INCLUDING GRANTS TO OVER 375 ORGANIZATIONS SERVING THE COUNTY, FROM AFTER-SCHOOL PROGRAMS TO FAITH-BASED ORGANIZATIONS TO HEALTH CARE PROVIDERS. HORIZON FOUNDATION HAS LAUNCHED SEVERAL NEW TRANSFORMATIVE PROGRAMS TO DEMONSTRABLY IMPROVE COMMUNITY OUTCOMES, INCLUDING EXPANDING ACCESS TO MENTAL HEALTH SERVICES, COVID-19 RESPONSE INITIATIVES, PROMOTING STREETS FOR ALL, INCREASING THE NUMBER OF RESIDENTS WHO HAVE AN IDENTIFIED HEALTH CARE AGENT, DRAMATICALLY REDUCING THE CONSUMPTION OF SUGARY DRINKS, REDUCING HEALTH DISPARITIES AND WORKING TOWARD RACIAL EQUITY IN HEALTH.
