EDUCATION PROGRAMS/EXIBIT TOURS - PEER MENTORING THAT OFFERS ONE-ON-ONE SUPPORT AND EDUCATION FOR THOSE NEWLY DIAGNOSED AND ALSO LONG-TERM SURVIVORS. SUSSEX/KENT COUNTY OUTREACH - PROGRAMS FOR VARIOUS POPULATIONS, LIAISONS BETWEEN HEALTHCARE AND COMMUNITY RESOURCES, ADDRESSING VARIOUS ISSUES RELATED TO DETECTION, DIAGNOSIS AND TREATMENT OF BREAST CANCER.
WOMEN'S HEALTH SCREENING - PROVIDES CENTRALIZED PATIENT NAVIGATION SERVICES TO HELP PATIENTS SCHEDULE AND OBTAIN SCREENINGS BY NAVIGATING PATIENTS THROUGH BARRIERS SUCH AS LACK OF TRANSPORTATION, LANGUAGE INTERPRETATION, INCOME RESTRICTIONS, OR LACKING A PRIMARY CARE PROVIDER. THE GOAL OF THIS PROGRAM IS TO INCREASE ACCESSIBILITY TO MAMMOGRAPHY AND OTHER HEALTH SCREENING SITE THROUGHOUT THE STATE TO REACH DIVERSE POPULATIONS AND INCREASE THE PERCENTAGE OF UNINSURED AND UNDERINSURED WOMEN RECEIVING CANCER SCREENING SERVICES IN DELAWARE.
