CDCHOICES AND PARTICIPATING EMPLOYING CONSUMERS HEALTH PLAN IS A VEBA TRUST ESTABLISHED FOR THE PURPOSE OF PROVIDING HEALTH, DENTAL, VISION, WORKERS COMPENSATION AND DISABILITY BENEFITS UNDER A FULLY-INSURED MULTIPLE EMPLOYER WELFARE ARRANGEMENT. PARTICIPATING EMPLOYERS INCLUDE CONSUMER DIRECTED CHOICES, INC. AND CONSUMERS OF NYS CONSUMER DIRECTED PERSONAL ASSISTANCE WHO CONTRACT WITH CONSUMER DIRECTED CHOICES FOR FISCAL INTERMEDIARY SERVICES. THE PLAN IS AVAILABLE TO THE EMPLOYEES OF CONSUMER DIRECTED CHOICES AND THE INDIVIDUAL EMPLOYING CONSUMERS WHO USE CONSUMER DIRECTED CHOICES. THE PLAN WAS TERMINATED AS OF DECEMBER 31, 2022.
