CONCORDIA PHYSICIAN PRACTICE PROVIDES MEDICAL SERVICES TO THE RESIDENTS OF CONCORDIA LUTHERAN HEALTH AND HUMAN CARE'S (EIN 25-0969458) INDEPENDENT LIVING, PERSONAL CARE, AND SKILLED NURSING AND REHABILITATION FACILITIES AS WELL AS THE SURROUNDING COMMUNITY. THE ORGANIZATION PROVIDES APPROPRIATE CARE TO PATIENTS THAT ALLOWS THEM TO STAY IN THE LEAST RESTRICTIVE, LEAST COSTLY LEVEL OF CARE IN THE CONCORDIA CONTINUUM OR WITHIN THEIR HOMES AS APPLICABLE, AS WELL AS ALLOWING FOR AT HOME VISITS TO LIMIT HOSPITAL STAYS FOR PATIENTS. THE ORGANIZATION PROVIDES CARE THE PATIENTS WHO ARE COVERED BY MOST MAJOR INSURANCE PLANS COMMONLY ACCEPTED IN THE REGION, INCLUDING MEDICARE AND PENNSYLVANIA MEDICAL ASSISTANCE FOR THE INDIGENT. THE PRACTICE'S STAFF PROVIDES CARE TO ITS PATIENTS, REGARDLESS OF THEIR ABILITY TO PAY. DURING THE FISCAL YEAR, CPP PROVIDED 9,884 SERVICE VISITS TO RESIDENT AND COMMUNITY MEMBERS AS WELL AS $981 OF BENEVOLENT CARE TO EIGHT RESIDENTS OF SKILLED NURSING, PERSONAL CARE, AND HOSPICE OF AFFILIATE ORGANIZATIONS. THE ORGANIZATION ALSO OFFERED A MEDICAL OFFICE ADMINISTRATION PRACTICUM, OR INTERNSHIP, TO A STUDENT AT A LOCAL COLLEGE. THROUGH THE OPERATIONS OF THE ORGANIZATION, CONCORDIA RESIDENTS AND THE RESIDENTS OF THE SURROUNDING COMMUNITY RECEIVE QUALITY CARE THAT INCREASES THEIR QUALITY OF LIFE AND THEIR ABILITY TO STAY IN THE LEAST RESTRICTIVE ENVIRONMENT FOR THE LONGEST POSSIBLE TIME.
THE ORGANIZATION ALSO PARTNERS WITH CONCORDIA LUTHERAN HEALTH AND HUMAN CARE (EIN 25-0969458)TO PROVIDE MEDICAL DIRECTOR SERVICES TO THE SKILLED NURSING, OUTPATIENT REHABILITATION, PERSONAL CARE, AND HOSPICE UNITS ON THEIR CABOT, PA CAMPUS. THROUGH THE MEDICAL DIRECTOR SERVICES, THE ORGANIZATION PROVIDES GUIDANCE AND LEADERSHIP TO THE OPERATIONS OF CONCORDIA.
