MAKING CHOICES MICHIGAN'S (MCM) MISSION IS TO ENCOURAGE AND FACILITATE ADVANCE HEALTH CARE PLANNING (ACP) BY THE PEOPLE OF MICHIGAN, WITH THE GOAL OF HAVING INDIVIDUALS DISCUSS THE CARE THEY WOULD CHOSE IF THEY ARE UNABLE TO SPEAK FOR THEMSELVES. IN 2019, MCM CONTINUED TO EXPAND ITS COMMUNITY OUTREACH EFFORTS THROUGH THE INTERFAITH ACP PROGRAM. IN ADDITION TO MCM'S USUAL OUTREACH ENDEAVORS, THE MCM-TRAINED FIRST STEP FACILITATORS PRESENTED ACP EDUCATION TO THEIR CONGREGANTS, FOLLOWED BY FACILITATED ACP CONVERSATION WITH WILLING PARTICIPANTS. BY 2019 YEAR-END, MCM VOLUNTEER FACILITATORS AND THE INTERFAITH FACILITATORS HAD EDUCATED 1,734 INDIVIDUALS, RECEIVED 84 CONVERSATION REQUESTS, FACILITATED 75 CONVERSATIONS, AND COMPLETED 65+ ADVANCE DIRECTIVES (AD). IN ADDITION, MCM REFERRED 3 CONVERSATIONS TO LOCAL SPANISH-SPEAKING OR OUTSIDE HEALTHCARE PROVIDER FACILITATORS. FINALLY, MCM TRAINED 52 NEW FIRST-STEP AND 8 ADVANCED STEP FACILITATORS IN 2019. OF NOTE, THE MAJORITY ARE STILL WORKING PART OR FULL TIME IN THEIR CHOSEN PROFESSION AND SEE MCM AS AN IMPORTANT COMMUNITY INITIATIVE WORTHY OF THEIR VOLUNTEER HOURS.
