MISSOURI MEDICAID EQUITY PROJECT. COMMUNITY HEALTH OUTREACH AND EDUCATION TO INCREASE ENROLLMENT OF NEWLY ELIGIBLE ADULTS IN EXPANDED MEDICAID AND IMPROVE INDIVIDUAL ACCESS TO CARE THROUGHOUT THE STATE OF MISSOURI.
PROVIDE QUALITY AFFORDABLE HOUSING, FOOD AND PERSONAL PROTECTIVE EQUIPMENT TO LOW INCOME INDIVIDUALS LIVING WITH CHRONIC DISEASES AND OTHER LONG TERM DISABILITIES IN ORDER TO IMPROVE POSITIVE LONG TERM HEALTH OUTCOMES.
