INDIVIDUALIZED SUPPORTED LIVING (ISL) - ISL HOMES ARE DESIGNED TO SERVE ONE TO FOUR INDIVIDUALS AND ARE LOCATED IN RESIDENTIAL, SINGLE-FAMILY NEIGHBORHOODS. A SUPERVISOR COORDINATES THE INDIVIDUALS' CARE, STAFF SCHEDULES, AND COMMUNITY INTEGRATION AS WELL AS STAFF SUPPORT. INDIVIDUALS LIVING IN ISL HOMES ARE PROVIDED UP TO 24 HOURS PER DAY SUPPORT BASED UPON THE NEEDS OF THE INDIVIDUAL(S). THE GOAL IS TO ENABLE THE INDIVIDUALS TO LIVE SAFE, HEALTHY LIVES IN A CLEAN ENVIRONMENT WHILE PROMOTING INDEPENDENCE AND INTEGRATION INTO THE LOCAL COMMUNITY.
A HOST HOME PROGRAM AFFORDS THE OPPORTUNITY FOR A PERSON WITH A DISABILITY TO CHOOSE TO LIVE WITH AN INDIVIDUAL, COUPLE, OR A FAMILY IN THE COMMUNITY TO SHARE THEIR LIFE EXPERIENCES TOGETHER. THE INDIVIDUAL THUS BECOMES A VALUED MEMBER OF THE HOME/FAMILY AND DEVELOPS PERMANENT RELATIONSHIPS THAT THEN HELP THAT INDIVIDUAL GROW AND DEVELOP. THE FAMILY RECEIVES A MONTHLY STIPEND PLUS ROOM AND BOARD FOR SUPPORTING THE INDIVIDUAL WITH DISABILITIES IN THEIR HOME. CHARITON VALLEY PROFESSIONAL MANAGEMENT STAFF OVERSEES SERVICES, ENSURES COMPLIANCE WITH STATE AND FEDERAL GUIDELINES, AND PROVIDES ON-GOING SUPPORT TO THE FAMILY AND INDIVIDUAL(S).
INDEPENDENT LIVING SKILLS DEVELOPMENT (ILSD) - TYPES OF SERVICES AND SUPPORTS PROVIDED INCLUDE SKILL TRAINING IN ACTIVITIES OF DAILY LIVING INCLUDING BUT NOT LIMITED TO: HYGIENE, MENU PLANNING, MEAL PREPARATION, HOME ENVIRONMENT, LAUNDRY, BUDGETING, AND COMPARISON SHOPPING. TRANSPORTATION ASSISTANCE IS ALSO PROVIDED FOR ESSENTIAL ERRANDS, SUCH AS DOCTOR'S APPOINTMENTS, GROCERY-SHOPPING, STOPS AT THE PHARMACY AND OFTEN TO AND FROM WORK.
