TO PROVIDE HOUSING AND SUPPORT SERVICES FOR 14-18 ADULTS WITH BRAIN INJURIES, MENTAL HEALTH CONDITIONS AND/OR SUBSTANCE USE DISORDERS, IN ADDITION TO A VARIETY OF HEALTH CONCERNS. ACTIVITIES FOCUS ON COMMUNITY INTEGRATION, SKILLS BUILDING, AND OVERALL WELLNESS. WE WORK COLLABORATIVELY WITH SOCIAL AND MEDICAL SERVICES TO PROVIDE PERSON-CENTERED CARE FOR INDIVIDUALS WE SUPPORT. WE STRIVE TO SUPPORT BRAIN INJURY AND MENTAL HEALTH AWARENESS AND PREVENTION THROUGHOUT THE COMMUNITY.
