CASE MANAGERS WORK WITH HIV POSITIVE CLIENTS TO ASSIST THEM IN OVERCOMING BARRIERS TO THEIR HEALTH AND WELL-BEING; THIS INCLUDES (BUT IS NOT LIMITED TO) ARRANGING TRANSPORTATION TO MEDICAL APPOINTMENTS, EMERGENCY FINANCIAL AID, HELP WITH BUDGETING, AND GUIDANCE TO OTHER AVAILABLE SUPPORT SERVICES.
OUTREACH TO IDENTIFY AT-RISK POPULATIONS AND INDIVIDUALS, HIV TESTING, COUNSELING AND REFERRAL TO HIV AND OTHER SERVICES IF NEEDED. PREVENTION SPECIALISTS WORK WITH GROUPS AND INDIVIDUALS TO REDUCE RISK OF TRANSMISSION OF HIV, AND TO PROMOTE OVERALL HEALTHY LIFESTYLES.
