INNOVAGE OPERATES A PACE PROGRAM (PROGRAM OF ALL INCLUSIVE CARE FOR THE ELDERLY) PROVIDING ADULT DAY CARE, CLINICAL SERVICES, HOME HEALTH, TRANSPORTATION, AND OTHER HEALTHCARE RELATED SERVICES TO FRAIL ELDERLY WHO ARE DEEMED NURSING HOME ELIGIBLE. IN FISCAL YEAR 2015, INNOVAGE SERVED AN AVERAGE OF 384 CLIENTS PER MONTH IN THE PACE PROGRAM. INNOVAGE ALSO PROVIDES SUPPORT AND COMMUNITY OUTREACH TO A FAMILY OF NON-PROFIT ORGANIZATIONS WHOSE COLLECTIVE MISSION IS TO HELP ELDERLY, FRAIL, AND DISABLED INDIVIDUALS LIVE WITH DIGNITY IN THEIR HOMES AND COMMUNITIES.
