HOME HEALTH CARE PROVIDES AT HOME MEDICAL SERVICES SUCH AS NURSING CARE AND THERAPY, UNDER ORDERS FROM THE PATIENT'S PERSONAL PHYSICIAN, TO HELP PATIENTS RECUPERATE SAFELY FROM ILLNESS OR INJURY. 1,083 PERSONS WERE SERVED DURING THE YEAR.
CASE COORDINATION UNIT CARE COORDINATORS, UNDER AN ILLINOIS DEPARTMENT ON AGING PROGRAM, PROVIDE ASSESSMENTS AND RESOURCE REFERRALS TO ASSIST ELDERLY, LOW INCOME ADULTS WITH GETTING THE SERVICES THEY NEED TO MAINTAIN A SAFE LEVEL OF INDEPENDENCE WHILE LIVING IN THEIR OWN CHOSEN PLACE OF RESIDENCE AS LONG AS POSSIBLE. 10,100 PERSONS WERE SERVED DURING THE YEAR.
