PROGRAM ADMINISTERED TO REDUCE PREMATURE DEATH AND DISABILITY IN AT-RISK POPULATIONS BY PROVIDING CLINICALLY ACCURATE AND INDIVIDUALLY DIFFERENTIATED PREVENTION, INTERVENTION AND POST-INTERVENTION SERVICES THROUGH VOLUNTEER AND PROFESSIONAL STAFF, IN COLLABORATIVE EFFORTS WITH COMMUNITY AND FAMILIES, USING AN OUTREACH MODEL IN KANE COUNTY, ILLINOIS.
SCHOOL OUTREACH PROGRAMS. SPS HAS CONTRACTS WITH SEVERAL KANE AND KENDALL COUNTY SCHOOLS FOR OUTREACH TO CHILDREN AS WELL AS BEING AVAILABLE IN CRISIS CONTROL AND INTERVENTION.
