CASE MANGEMENT: ASSISTS CONSUMERS ON TRAUMATIC BRAIN INJURY (TBI) WAIVERS AND COMMUNITY ALTERNATIVE FOR DISABLED INDIVIDUALS (CADI) WAIVERS, AND PROVIDES RELOCATION SERVICE COORDINATION WITHIN CONTRACTED COUNTIES AND OFFERS FLEXIBLE CASE MANAGEMENT SERVICES FOR CONSUMERS UTILIZING CONSUMER DIRECTED COMMUNITY SUPPORTS (CDCS).
