THE MISSION OF CHAP IS TO ELIMINATE HEALTH AND SOCIAL DISPARITIES IN OUR COMMUNITY BY FINDING THOSE AT RISK, CONNECTING THEM TO CARE, AND MEASURING THE OUTCOMES. WE BELIEVE ALL COMMUNITIES CAN BE TRANSFORMED THROUGH THE WORK OF COMMUNITY HEALTH WORKERS AND THE CREATION OF COMMUNITY HUBS AN ACCOUNTABLE CARE COORDINATION DELIVERY SYSTEM.

Part 3 - Line 4a

HUB DEVELOPMENT PROJECT: CHAP HUB IS A NATIONALLY LEVEL 2 CERTIFIED PATHWAYS HUB. THE HUB CONTRACTS WITH CARE COORDINATION AGENCIES (CCAS) THAT PROVIDE SERVICES TO INDIVIDUALS AT-RISK FOR POOR OUTCOMES. CHAP HUB COVERS ASHLAND, CRAWFORD, HURON, KNOX, MORROW AND RICHLAND COUNTIES IN OHIO. COMMUNITY HEALTH WORKERS (CHWS) ARE EMPLOYED BY CCAS TO PROVIDE CARE COORDINATION AND THE HUB CONTRACTS WITH PAYERS TO PROVIDE PAYMENT FOR THESE SERVICES. CHAP HUB CONTINUES TO EXPAND COVERAGE TO AT-RISK INDIVIDUALS LIVING IN THESE COUNTIES.


DIRECT CLIENT SERVICES: INCLUDED IN DIRECT CLIENT SERVICES ARE CHAP'S HELP ME GROW (HMG), MOMS AND BABIES FIRST (MBF) AND UNITED WAY PROGRAMS ALONG WITH ITS INVOLVEMENT IN OHIO MEDICAID MANAGED CARE PLANS (MCPS). MEDICAID MCPS IN OHIO CONTRACT WITH CHAP HUB TO PROVIDE CARE COORDINATION SERVICES TO THEIR MEMBERS AT RISK OF POOR OUTCOMES. HMG IS A PROGRAM FOR EXPECTANT PARENTS, NEWBORNS, INFANTS AND TODDLERS THAT USES HOME VISITS TO PROMOTE HEALTHY GROWTH AND DEVELOPMENT. MBF PROVIDES CARE COORDINATION SERVICES TO AFRICAN-AMERICAN AND OTHER MINORITY WOMEN AT RISK FOR POOR BIRTH OUTCOMES AND MONITORS THEIR INFANTS THROUGHOUT THE FIRST YEAR OF LIFE. UNITED WAY IN RICHLAND COUNTY PROVIDES FUNDING FOR CARE COORDINATION SERVICES FOR INDIVIDUALS THAT ARE NOT COVERED UNDER MEDICAID OR ELIGIBLE FOR THE HMG OR MBF PROGRAMS.
