SUCH ORGANIZATIONS ARE ENGAGED IN THE PROVISION OF HOME HEALTH SERVICES, INPATIENT AND HOME HOSPICE SERVICES, THE PROVISION OF DURABLE MEDICAL EQUIPMENT AND HOME INFUSION SERVICES.PURSUANT TO A RESTRUCTURING PLAN APPROVED BY ITS BOARD OF DIRECTORS, THE ORGANIZATION WAS MERGED INTO ONE OF ITS SUPPORTED ORGANIZATIONS, VISITING NURSE SERVICE, INC.
