TRANSITIONAL LIVING, INC PROVIDES COMPREHENSIVE MENTAL HEALTH SERVICES TO ADULTS WHO HAVE EEN DIAGNOSED WITH DISORDERS SUCH AS SCHIZOPHRENIA, BIPOLAR DISORDER AND MAJOR DEPRESSIVE DISORDER. CONSUMERS AND STAFF WORK TOGETHER TO CREATE A PLAN OF CARE THAT MAY INCLUDE PSYCHIATRIC SERVICES, CASE MANAGEMENT SERVICES, VOCATIONAL SERVICES AND RESIDENTIAL SUPPORT THROUGH COMPREHENSIVE MENTAL HEALTH SERVICES. THE ORGANIZATION SEEKS TO ASSIST INDIVIDUALS WITH THE FOLLOWING OUTCOMES - INCREASING QUALITY OF LIFE, DECREASING SYMPTOM DISTRESS, INCREASING INDEPENDENT LIVING SKILLS, LINKING TO COMMUNITY RESORCES, RECONNECTING TO NATURAL SUPPORTS SUCH AS FAMILY AND FRIENDS. EACH EARY, OVER 800 CLIENTS RECEIVE MENTAL HEALTH SERVICES. SPECIALIZED SERVICES ALSO FOCUS ON THOSE WHO HAVE BEEN DIAGNOSED WITH MENTAL ILLNESS AND ARE HOMELESS, OR CONNECTED TO THE CRIMINAL JUSTICE SYSTEM OR COPING WITH FREQUENT HOSPITALIZATIONS. TRANSITIONAL LIVING, INC. ALSO HAS TAKEN A MAJOR COMMUNITY ROLE IN TRAINING INDIVIDUALS IN THE COMMUNITY WHO SERVE OR HAVE FREQUENT CONTACT WITH OUR CONSUMERS OR POTENTIAL CONSUMERS INCLUDING LAW ENFORCEMENT, JAILS, FIRE DEPARTMENTS AND PARAMEDICS, HEALTH CLINICS, HOMELESS PROVIDERS AND CHURCHES. TRANSITIONAL LIVING, INC. PATH TEAM (HOMELESS OUTREACH) HAS ALSO PRESENTED AT NATIONAL CONFERENCES SUCH AS THE GAINS CONFERENCE BOTH IN BOSTON AND WASHINGTON DC.
