DAYBREAK PROVIDED HABILITATIVE SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES WHO ARE IN THE MEDICAID WAIVER PROGRAM. DURING FISCAL 2021, DAYBREAK PROVIDED 31,807 DAY HABILITATION SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES. A UNIT OF SERVICE IS MEASURED BY A SET NUMBER OF HOURS AN INDIVIDUAL SPENDS AT ONE OF THE ORGANIZATION'S DAY HABILITATION SITES WORKING ON DAILY ACTIVITIES AS WELL AS INTEGRATION IN THE COMMUNITY.
DAYBREAK PROVIDED HABILITATIVE SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES WHO LIVE IN THEIR OWN HOME OR FAMILY HOME. DURING FISCAL 2021, DAYBREAK PROVIDED 31,166 COMMUNITY HABILITATION SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES.
