GRACE MEDICAL HOME SERVES AS A VITAL LINK IN CENTRAL FLORIDA'S HEALTHCARE SAFETY NET SYSTEM BY PROVIDING AN EXCEPTIONAL MEDICAL PRACTICE TO SERVE THE HOLISTIC NEEDS OF THE LOW INCOME, UNINSURED OF OUR COMMUNITY. OUR PATIENTS ARE UNINSURED ADULTS AND CHILDREN IN ORANGE COUNTY WHO RESIDE AT OR BELOW 200 PERCENT OF THE FEDERAL POVERTY LEVEL (FPL). ONCE ENROLLED, PATIENTS HAVE ACCESS TO PRIMARY AND PREVENTIVE CARE, IN-HOUSE LABS, DIGITAL X-RAY, VACCINATIONS, DIABETES EDUCATION, DONATED MEDICATIONS, TREATMENT OF ACUTE AND CHRONIC ILLNESSES, AND ON-SITE MEDICAL SPECIALTIES INCLUDING CARDIOLOGY, MENTAL HEALTH COUNSELING, DENTAL, ENDOCRINOLOGY, OPHTHALMOLOGY, ORTHOPEDIC, AND MORE. WHAT DISTINGUISHES GRACE IS RECOGNITION FOR PROVIDING THIS UNPARALLELED LEVEL OF CARE AT THE LOCAL, STATE AND NATIONAL LEVEL. IN 2023, THE ORGANIZATION SERVED 1,276 UNDUPLICATED PATIENTS. THESE PATIENTS HAD MORE THAN 16,433 PATIENT ENCOUNTERS, RECEIVED MORE THAN $13,973,235 IN VALUE OF CARE, INCLUDING PRESCRIPTION MEDICATIONS OBTAINED ON BEHALF OF SPECIFIC PATIENTS OR DONATED DIRECTLY TO OUR FACILITY, AND WERE OFFERED 35 MEDICAL SPECIALTIES ON-SITE. IN THE FALL, WE LAUNCHED OUR MOBILE MEDICAL UNIT TO UNDERSERVED NEIGHBORHOODS AND HAD 163 PATIENT VISITS ON THE UNIT, ELIMINATING THE TRANSPORTATION BARRIER THAT EXISTS FOR MANY OF OUR PATIENTS. THANKS TO THE 510 VOLUNTEERS AND STAFF, WE PROVIDED 1,879 MENTAL HEALTH VISITS, 1,837 DENTAL VISITS, 372 SOCIAL SERVICE VISITS, SCREENED 922 PATIENTS FOR SPIRITUAL WELLNESS, AND DISTRIBUTED 1,843 FOOD BOXES TO FOOD-INSECURE PATIENTS.
