GRACE MEDICAL HOME SERVES AS A VITAL LINK IN CENTRAL FLORIDA'S HEALTHCARE SAFETY NET SYSTEM BY PROVIDING AN EXCEPTIONAL MEDICAL PRACTICE TO SERVE THE ONGOING, PRIMARY AND PREVENTIVE MEDICAL CARE, ORAL HEALTH AND MENTAL HEALTH NEEDS OF THE LOW INCOME, UNINSURED OF ORANGE COUNTY, AS WELL AS COMPREHENSIVE CHRONIC DISEASE MANAGEMENT THAT DEMONSTRATE IMPROVED CLINICAL OUTCOMES. GRACE IS NOT JUST A CLINIC THAT PROVIDES COMPREHENSIVE CARE TO THE UNINSURED FOR LITTLE OR NO COST, BUT A MEDICAL HOME PROVIDING CONTINUOUS, COMPREHENSIVE PRIMARY AND PREVENTIVE CARE. PATIENTS HAVE AN ONGOING AND PERSONAL RELATIONSHIP WITH THEIR PROVIDER AND A PLACE TO TURN TO FOR THEIR HEALTHCARE NEEDS. IT IS PRECISELY WHAT THE LOW-INCOME, UNINSURED IN ORANGE COUNTY ARE UNABLE TO ACCESS DUE TO COST-BARRIERS AND INSURANCE STATUS. BY GOD'S GRACE, THE ORGANIZATION EXTENDS HOPE, HEALING, AND WHOLENESS THROUGH MEANINGFUL RELATIONSHIPS WITH PATIENTS. THE ORGANIZATION'S SERVICES INCLUDE PRIMARY AND PREVENTATIVE CARE, IN-HOUSE LABS, DIGITAL X-RAY, VACCINATIONS, DIABETES EDUCATION, ACCESS TO DONATED MEDICATIONS, TREATMENT OF ACUTE AND CHRONIC ILLNESSES, AND ON-SITE MEDICAL SPECIALTIES INCLUDING CARDIOLOGY, MENTAL HEALTH COUNSELING, DENTAL, ENDOCRINOLOGY, OPHTHALMOLOGY, VASCULAR, AND MORE. IN 2021, THE ORGANIZATION ENROLLED 253 NEW PATIENTS DESPITE A GLOBAL PANDEMIC (165 ADULT AND 88 CHILDREN) FOR A TOTAL OF 1,207 UNDUPLICATED PATIENTS. THESE PATIENTS HAD MORE THAN 16,623 PATIENT ENCOUNTERS, RECEIVED MORE THAN $10,358,000 IN VALUE OF CARE, INCLUDING PRESCRIPTION MEDICATIONS OBTAINED ON BEHALF OF SPECIFIC PATIENTS OR DONATED DIRECTLY TO OUR FACILITY, AND WERE OFFERED 35 MEDICAL SPECIALTIES ON-SITE, INCLUDING 2,653 DENTAL CENTER VISITS AND 1,710 MENTAL HEALTH OR SPIRITUAL CARE VISITS. IN FACT, 18% OF GRACE PATIENTS RECEIVED MENTAL HEALTH SERVICES, A 35% INCREASE FROM 2020. THE ORGANIZATION'S MEDICAL OUTCOMES FOR CHRONICALLY-ILL PATIENTS DEMONSTRATE AN IMPROVEMENT IN PEOPLE'S HEALTH HELPING THEM REMAIN ON THE PATH OF SELF-SUSTAINABILITY. AND IN A TIME OF ECONOMIC FRAGILITY FOR PATIENTS, THE ORGANIZATION EXPANDED THE FOOD IS MEDICINE PROGRAM AND BECAME AN OFFICIAL CLOSED FOOD PANTRY AND SECURED FUNDING TO INCORPORATE HEALTHY FOOD WITH OUR PATIENT'S TREATMENT PLAN. EVERY OTHER WEEK, ELIGIBLE PATIENTS DRIVE THROUGH TO PICK UP HEALTHY FOOD BAGS WILLED WITH PROTEINS, HEALTHY GRANS OR OATS, FRESH FRUITS, AND VEGETABLES TO BUILD THE FRAMEWORK FOR A HEALTHIER LIFESTYLE.
