COMMUNITY BASED SERVICES - PROVIDES MEDICAL & CASE MANAGEMENT SERVICES, BEHAVIORAL HEALTH COUNSELING, FULL TRANSPORTATION, SUPPORT GROUPS & VARIOUS SOCIAL ACTIVITIES, AND EDUCATIONAL SEMINARS TO HIV+ PERSONS.
