HOSPICE PROGRAM: CHPC PROVIDES COMPASSIONATE, EXPERT END-OF-LIFE CARE TO RESIDENTS OF CHAUTAUQUA COUNTY, NEW YORK. HOSPICE SERVICES ARE AVAILABLE TO PATIENTS WHEREVER THEY CALL HOME, INCLUDING THEIR OWN HOME, AN ASSISTED LIVING FACILITY, A NURSING HOME, OR HOSPITAL
BEREAVEMENT SERVICES: CHPC PROVIDES BEREAVEMENT SERVICES THROUGH HOSPICE BEREAVEMENT SUPPORT, COMMUNITY BEREAVEMENT SUPPORT, AND GRIEF SUPPORT GROUPS.
