FUNDRAISING PROGRAMS: THE FOUNDATION EXISTS TO SUPPORT MEMBERS OF THE HEALTHCARE SYSTEM OF LANCASTER GENERAL HEALTH. THE FOUNDATION ENGAGES IN FUNDRAISING AND OTHER ACTIVITIES IN ORDER TO PROVIDE FINANCIAL SUPPORT TO THE SUPPORTED ORGANIZATIONS AND TO ADMINISTER AND INVEST FUNDS FOR THEIR BENEFIT, ALL IN ORDER TO ASSIST THEM IN CARRYING OUT THEIR IRC SECTION 501(C)(3) CHARITABLE HEALTH MISSIONS BY ENHANCING THE DELIVERY OF HEALTH CARE SERVICES AND IMPROVING COMMUNITY HEALTH.

PURPOSE: Lancaster General Hospital: 23-1365353 The Lancaster General Hospital Population Health Fellowship program award of $123,000 was given in order to develop a network of physicians in leading the move to Accountable Care in Lancaster County. The grant will enable the felllowship to provide additional education to physicians, and increase access to care for the most challenging patient population. The Lancaster General Hospital Neuroscience Back Pain program received funding of $236,536 to deliver a standardized ambulatory approach for high volume condition - acute low back pain. The target popluation includes adults with acute onset of low back pain in the outpatient community. The program's initiative is to provide improved patient care outcomes while reducing per capita costs of care.
