RURAL HEALTH NETWORK PROGRAMS:HCHN'S ACTIVITIES UNDER ITS NEW YORK STATE DEPARTMENT OF HEALTH RURAL HEALTH NETWORK FUNDING SUPPORTS THE HEALTH AND WELL-BEING OF INDIVIDUALS WHO LIVE, WORK, PLAY AND LEARN IN HERKIMER COUNTY AND THE MOHAWK VALLEY BY ADDRESSING THE FOLLOWING PRIORITY AREAS: ACCESS TO QUALITY HEALTHCARE, CHRONIC DISEASE PREVENTION AND SELF-MANAGEMENT, HEALTHY COMMUNITIES, OBESITY PREVENTION, ACCESS TO HEALTHY FOOD, PHYSICAL ACTIVITY AND NUTRITION. HIGHLIGHTED ACTIVITIES UNDER THESE PRIORITY AREAS INCLUDE: SPONSORS MEDICAL TRANSPORTATION PROGRAMS, CHILD DENTAL HEALTH EDUCATION, ESTABLISHMENT OF MOHAWK VALLEY MILE WALKING TRAILS, INCREASED ACCESS TO VEGETABLE/HEALTHY FOODS IN COMMUNITY MARKETS, DISEASE SELF-MANAGEMENT CLASSES AND NUTRITION SEMINARS.
