OFFICE OF RURAL HEALTH QUALITY IMPROVEMENT GRANT: PROGRAM THAT HAS ALLOWED UNIQUE PRIMARY CARE CLINICS TO MERGE THEIR DATA INTO ONE SYSTEM FOR THE PURPOSES OF DOING POPULATION WIDE, PRACTICE SPECIFIC, PROVIDER SPECIFIC AND PATIENT SPECIFIC REPORTING. THOSE REPORTS PROVIDE A BASIS FOR DEVELOPING QUALITY IMPROVEMENT PLANS FOR IMPLEMENTATION. ENABLED BY THE NECESSITY OF MORE VIRTUAL INTERACTION DUE TO COVID-19, DELIBERATE PROGRESS AND EFFICIENCIES HAVE BEEN ACHEIVED IN TRANSITIONING FROM FACE-TO-FACE INTERACTIONS WITH GEOGRAPHICALLY DISPERSED PARTNERS AND PRACTICES TO ON-LINE WORKING GROUP. MORE PROACTIVE AND INNOVATIVE EFFORTS HAVE LED TO MORE PRELIMINARY CONSULTATIONS WITH LOCAL FOUNDATIONS, LOCAL CBOS AND LOCAL/COUNTY GOVERNMENT SIGNALING A MORE INTEGRATED APPROACH TO ADDRESS SOCIAL DETERMINANTS OF HEALTH IN THE FUTURE, WHICH IS MORE RESILIENT TO NEW CHALLENGES AND LESS DEPENDENT ON EXTERNAL FUNDING.
CREATING HEALTHY SCHOOLS AND COMMUNITIES: PROGRAM TO HELP PREVENT OBESITY, TYPE 2 DIABETES, AND OTHER CHRONIC DISEASES. THIS PROGRAM ADDRESSES THE ONGOING NEED FOR SUSTAINABLE POLICY, ENVIRONMENT, AND SYSTEMS CHANGES TO SUPPORT BETTER PHYSICAL AND NUTRITIONAL HEALTH. CHSC ASSISTED SIX SCHOOL DISTRICTS TO ADOPT LOCAL WELLNESS POLICIES AND HOSTED TWO COMMUNITY CONVERSATIONS. PLANNING IN PROGRESS FOR THE CHADAKOIN ADVENTURE TARIL LINKING JAMESTOWN ELEMENTARY SCHOOL TO NEARBY PARK WITH IMPROVED STRUCTURES AND EDUCATIONAL SIGNAGE ALONG THE WAY.
CHAUTAUQUA REGION HEALTH SERVICE STRENGTHENING STRATEGY PROJECT IS AN EFFORT BY THE CHAUTAUQUA HEALTH NETWORK TO: 1. IDENTIFY KEY OBSTACLES WITHIN THE PRIVATE, PUBLIC AND NOT-FOR-PROFIT SECTORS TO ACHIEVING BETTER POPULATION HEALTH AND TO; 2.
